Before your visit

Request an appointment

Book online or call (503) 461-4005. Meredith personally reviews every request and confirms within one business day. You'll receive a confirmation email with your appointment time.

Complete your intake forms

After your appointment is confirmed, you'll get a link to fill out intake forms online, from home, on your own time. These cover your medical history, current symptoms, and what you've already tried. It takes most people 15-20 minutes.

What to have ready

  • A list of your current medications and doses, if any.
  • Names of other providers you're working with (therapist, PCP), if you'd like Meredith to coordinate with them.
  • A quiet, private space for your video visit.
  • A location in Oregon. You must be physically in the state at the time of your appointment.

During your visit

Your first visit is a real conversation: a full 60 minutes. Meredith will take the time to understand the full picture before making any recommendations.

What Meredith will ask about

  • What's been going on and what brought you here now.
  • Your symptoms: when they started, what makes them better or worse.
  • Your medical history and any medications you've tried.
  • Your life context (sleep, stress, relationships, work), because symptoms don't exist in a vacuum.

What happens next in the visit

After listening to the full picture, Meredith will explain what she's seeing and talk through options with you. If medication makes sense, she'll explain what she'd recommend and why: the expected benefits, possible side effects, and what to watch for. If medication doesn't make sense right now, she'll say that too.

Medication is always a conversation, never a foregone conclusion.

You won't be pressured. You'll make decisions together. Some patients start medication at their first visit. Some want to think about it. Some decide medication isn't right for them. All of that is fine.

After your visit

If you start medication

Meredith will send any prescriptions electronically to your pharmacy. She'll schedule a follow-up visit (usually 2-4 weeks out) to check in on how things are going. If something doesn't feel right before then, you can reach out. You're not on your own between visits.

Follow-up visits

Follow-ups are 30 minutes, $250. That's enough time to actually talk about how things are going, and Meredith adjusts the plan as needed. Psychiatric medication management is a process, not a one-and-done prescription.

Coordination with your other providers

If you're working with a therapist or PCP, Meredith can coordinate with them with your written permission. Many patients receive therapy and medication management at the same time, from different providers, and coordination helps keep everyone working from the same plan.

Questions about first visits

I've never seen a psychiatric provider before. Is that okay?
Absolutely. Many people seeking care have never seen a mental health provider before. You don't need a diagnosis, a referral, or even a clear idea of what's wrong. That's what the first visit is for. Meredith will walk you through everything.
Is telehealth as good as in-person for psychiatric care?
Psychiatric medication management adapts well to telehealth because the visit is built around a clinical conversation. You have that conversation from your own home, anywhere in Oregon, and if an in-person visit ever becomes necessary, Meredith will discuss that with you.
What if the medication doesn't work?
Psychiatric medication management is a process. If the first approach isn't the right fit, Meredith will adjust the plan with you. Follow-up visits are 30 minutes, $250, enough time to actually talk about how things are going and make thoughtful changes.
What is a PMHNP?
A Psychiatric Mental Health Nurse Practitioner is a master's-or-doctoral-prepared provider who can diagnose, prescribe, and manage psychiatric medications. Meredith is board-certified (PMHNP-BC), with thirteen years in social work and ten in psychiatric nursing behind her prescribing.
