Getting started

What happens during the first visit?
Your first visit is 60 minutes by video. Meredith will ask about what's been going on, your history, and what you've already tried. You'll talk through options together. Medication is always a conversation, never a foregone conclusion.
Do I need a referral to book?
No referral is needed. You can book your first visit directly. Many patients come after a recommendation from a therapist or primary care provider, but it's not required.
How do I book an appointment?
Request your first visit through our website or call (503) 461-4005. Meredith personally reviews every appointment request and confirms within one business day.
What should I bring to my first visit?
You'll complete intake forms online before your visit, no clipboard in a waiting room. Have a list of current medications handy, and think about what you've already tried and what brought you here. That's it.

About the provider

What is a PMHNP?
A psychiatric-mental health nurse practitioner is a master's-or-doctoral-prepared provider who can diagnose, prescribe, and manage psychiatric medications. Meredith holds a PMHNP-BC certification from the American Nurses Credentialing Center, with thirteen years in social work and ten in psychiatric nursing behind her prescribing.
Will I see the same provider every time?
Yes. Meredith is your provider for every visit. She knows your name, your history, and your story, and you see her each time.

Pricing and insurance

How much does it cost?
First visit: $400 (60 minutes). Follow-up: $250 (30 minutes). Extended follow-up: $375 (60 minutes). You know what it costs before you book. No surprise bills.
Do you accept insurance?
We are out-of-network with all insurance plans. For patients with commercial insurance, we provide superbills (upon request) that you can submit for possible out-of-network reimbursement. Check your plan's out-of-network benefits. We are not able to see Medicare or Medicaid (Oregon Health Plan, or OHP) members at this time.
What is a superbill?
A superbill is a detailed receipt you submit to your insurance company for out-of-network reimbursement. We provide one (upon request). Your insurance company determines your reimbursement based on your plan's out-of-network benefits.
Why cash pay instead of insurance?
Cash pay keeps things simple: no prior authorizations, no insurance paperwork between you and booking, and no surprise bills from us. You get transparent pricing and longer visits, and you pay per visit at a price you know before you book. If you have commercial insurance and want to seek out-of-network reimbursement, we provide a superbill you can choose to submit. Reimbursement is determined by your plan, and your insurer may cover some, all, or none of it.

Telehealth

Is telehealth as good as in-person for this?
Psychiatric medication management adapts well to telehealth because the visit is built around a clinical conversation. You have that conversation from your own home, anywhere in Oregon: no waiting room, no drive. If an in-person visit ever becomes necessary, Meredith will discuss that with you.
What technology do I need?
A device with a camera and microphone (smartphone, tablet, or computer), a stable internet connection, and a private space where you can talk freely. On a computer, telehealth runs in your web browser with nothing to download. On a smartphone or tablet, you'll use the free Telehealth by SimplePractice app; you'll be prompted to download it when you join your first visit.
Do I have to be in Oregon?
Yes. You must be physically located in Oregon at the time of each visit. This is a state licensing requirement. We serve patients anywhere in the state.

Medication

Will I be prescribed medication on the first visit?
Not necessarily. Medication is a decision you make together with Meredith after a thorough evaluation. Some patients start medication on the first visit when the clinical picture is clear. Others take time to decide. There is no pressure either way.
What if I don't want to take medication?
That's a valid outcome. The first visit is an evaluation and a conversation. If you decide medication isn't right for you, Meredith can discuss alternatives and refer you to other resources.
What about side effects?
Meredith goes through every option including side effects so you can make an informed choice. The approach is start low, go slow, and adjust. You're not locked in. If something isn't working, you'll talk about it at your next follow-up.
Do I have to be on medication forever?
Not necessarily. How long medication is recommended depends on your symptoms, history, response to treatment, preferences, and clinical needs. The plan, including what stopping looks like, is part of the conversation from the beginning.
Can you prescribe controlled substances?
When clinically appropriate, yes. Prescribing follows established clinical guidelines and the Oregon Prescription Drug Monitoring Program, and a treatment agreement is signed at your first or second visit.

Working with other providers

I already have a therapist. Will you coordinate with them?
Yes. With your written permission, Meredith communicates directly with your therapist so both providers are working from the same plan.
Will my PCP be informed?
With your consent, Meredith coordinates directly with your primary care provider. We complement your PCP, not compete with them.
