However works for you. There's no required form and no wrong way to reach us: a short email with the client's name, best contact, and what's going on is plenty, or use the one-pager if you prefer it. We'll take it from there.

Your prescriber

Meredith Galvin, PMHNP-BC, is the founding provider at Coast to Coast Health Group. She worked at Duke Regional Hospital in inpatient psychiatry, and brings thirteen years of prior social work experience to a practice that treats patients across Oregon by telehealth.

One reason she started the practice: too many clinicians couldn't find a prescriber who would call them back or stay in the loop. Being reachable is built into how this practice runs. This page is that commitment, in writing.

How fast will I hear back after a visit?
You reach the prescriber directly, and coordination happens within one business day, after the first visit and whenever the plan changes. It works the way that suits your practice.
Will you talk to me about my shared client's treatment plan?
Yes. Coordination with consent is the default model. Treatment goals are aligned with your care plan, not treated as a separate track.

If you're reading this because your therapist sent you the link, here's what a first visit looks like. You meet with Meredith Galvin, PMHNP-BC, by video for 60 minutes. She'll ask about what's been going on, what you've tried, and what you're hoping for. Medication is one option, not a foregone conclusion. Our first visit is $400; follow-ups are $250. We are out of network with all insurance plans, and we provide superbills (upon request) for possible out-of-network reimbursement on commercial plans. We are not able to see Medicare or Medicaid (Oregon Health Plan, or OHP) members.

Your therapist stays your therapist. We coordinate with them on the medication side, with your consent.
